Patient concern form
If you wish to submit a patient concern, please do so in writing using our Patient Concern Form [PDF] so we may investigate, address and work with you to resolve your concern.
If you do not have a printer or do not wish to print the form, please send your concerns in an email to: dentalqa@louisville.edu.
Patient advisory board
As part of the School of Dentistry’s strategic plan to build patient-centered care, the school started a Patient Advisory Board in 2013. This patient group regularly meets with dental school leaders and students to discuss important topics related to improving the patient experience. These meetings benefit patients, as well as dental school students, faculty and staff.
Goals of the Patient Advisory Board include:
- Learn about priority concerns for patients, which may be different than that of the organization.
- Receive direct feedback from patients.
- Grow the School of Dentistry’s patient-centered culture.
- Strengthen community relations.
- Appreciate the barriers and opportunities for patients that were previously not understood.
- Identify system issues that need to be addressed in order to provide better patient-centered care.
- Achieve higher satisfaction ratings through collaboration in a patient-centered care model.
