Our dental student clinic has six comprehensive care teams, each with faculty supervisors and support staff such as schedulers and assistants. Once you become a patient, you will be assigned to one of these teams. If your dental needs are too complex for the student clinics, you may be referred to an advanced education program or to private practice to better suit your needs. Due to the educational nature of the treatment process and the level of faculty supervision required, please understand appointments in student clinics may be lengthier than in a private dental practice and may require multiple visits. Also, it very important that patients show up for appointments as scheduled because they play an important part in students' education and progress toward graduation. What to expect

Here's an overview of what to expect as a new patient in our dental student clinic:

Scheduling and preparation

  • Call 502-852-5096 to schedule an initial, 90-minute screening appointment. When you schedule, let us know if you'll need any accommodations.
  • If you have current dental records/X-rays, please email them to dentalca@louisville.edu or send them to our mailing address before your appointment.
  • Gather required information to bring to your appointment:
    • Payment for the non-refundable screening fee
    • Valid photo ID
    • Emergency contact information
    • Insurance/Medicaid card(s)
    • Your Health History, which is part of the New Patient Information packet (PDF)
      Current list of physicians and phone numbers and explanation of involvement in your care
    • Current list of medications

Your first visit

  • Arrive a few minutes early to register with our Patient Services Representatives. Be sure to allow time for parking.
  • Pay your non-refundable screening fee of $75. This amount covers the cost of one panoramic X-ray. If you do not become a patient at ULSD, the $75 is not refundable. However, you may request a copy of the panoramic X-ray image to take to any other provider. We accept cash, Visa, MasterCard, Discover and American Express payment for the first visit. We cannot accept checks.
  • A model or cast of your teeth will be created.
  • You will be asked questions about your Health History and current list of medications.
  • Your current records/X-rays from referring providers/ private dentists will be reviewed.
  • Please share any concerns and ask questions to ensure open communications and transparency during your treatment.

Please leave children at home or bring another adult to supervise them in the waiting area.

Your second and third visits

  • A comprehensive examination is completed in two separate appointments.
  • Based upon your initial screening, additional X-rays may be required to develop your treatment plan.
  • A cast or mold of your mouth is created.
  • The cost for these visits depends upon the number of additional diagnostic images needed, as well as clinic assignment and insurance coverage
  • You will be assigned to a clinic based upon the complexity of your needs, budget and personal preference.
  • A treatment plan will be developed and discussed with you, and  appointments scheduled.
  • Future DMD clinic visits can last up to 3 hours. Please plan accordingly.
  • Payment plans can be made.
